If you are calling with an urgent medical concern that cannot wait for the next business day, or if you need in emergency medications (not for controlled substances), have the pharmacy telephone number ready, call office at 813-415-0100 and wait for the further instructions.
Please contact our office at 813-415-0100 for all urgent appointments. Routine appointments and non-urgent appointments can be scheduled via Patient Portal once the user set up their own portal account. We encourage patients who will need follow-up appointments with DocCare Medical Clinic to schedule these when checking out at your current appointment to ensure all well ability of preferred dates and times.
Hours of operations
Monday to Friday 8:00 AM to 4:30 PM with lunch from 12 to 1 PM.
When you come to our office, you will be asked to register with a receptionist. Please notify the receptionist of any name change or addresses, or telephone number, or employment, or insurance information. Please provide us with the most current insurance information. New patients should be required to fill out the Patient History Form.
The office staff has been trained to answer any questions. If they’re unable to answer a question, they will relate the information to Dr. Patel and will call you back within reasonable time frame.
Our Office policies requires all prescription to be filled at visit. Please bring all your medication at every visit. Please note that we cannot call in any scheduled drugs (narcotics).
Your medical record is confidential and It will not be released without a written consent. You must sign a Medical Record Release form before we can release your medical records. There will be a charge for medical records according to Florida statue $25.00 for first 25 pages (subject to change). For additional questions, you may call the office at 813-415-0100.
