Patient Portal (My Medical Record)

Our Patient Portal is a secure way to communicate electronically with our patients. While DocCare Medical Clinic patients have access to a portal, we are always updating the websites. Please register for your Patient Portal account in the portal while following instructions by clicking on the “My Medical Record” link on home page. If you have not received an email invitation for setting up your portal account, please provide an update to your email address with our office at your next appointment. We will post the lab results and other test results in your portal account. The portal is a great way for patients to send non- urgent appointment requests, questions for your nurse, billing inquiries, refer request or questions and general inquiries.

In order to utilize a portal you must set up an account through which you will have secure communications. Portal account can only be set up for existing patient.


Please contact our office at 813-415-0100 for all urgent appointments. Routine appointments and non-urgent appointments can be scheduled via Patient Portal once the user set up their own portal account. We encourage patients who will need follow-up appointments with DocCare Medical Clinic to schedule these when checking out at your current appointment to ensure availability of preferred dates and times.

Cancel an appointment

We advise all patients to call our office or send a message via portal at least 72 hours prior to the appointment if you are unable to keep the appointment. We encourage that you keep the appointment since your preferred appointment time and date may not be available if we do not show up for the current appointment. It is our office policy that if one misses more than 3 consecutive appointments without any valid reasons we may terminate our relationship with you.

Insurance plans accepted

The following is a list of insurance company which we currently participate. If the insurance is not listed, it may be a commercial plan that does not require prior authorization. If you are unsure, you may contact your insurance provider to find out if they require to see a network physician or participating provider. In all cases, we’ll follow the insurance provider as a courtesy for our patients. Patients are required to pay any deductibles or co-insurances at the time of service. The deductible portion will be billed after we get the payment notification from the insurance carrier.

Lab results

We will forward the lab results electronically to you if you have created Patient Portal account. Please be sure to register for your Patient Portal account if you have not already have one in order to review your labs. Please register now from our home page. Lab results will be posted 24 hours after Dr. Patel has reviewed them. If the results are abnormal or medication change or additional tests needed in response to your labs, Dr. Patel’s nurse or the medical assistant will contact you by telephone to review the results with you. You should expect to receive a call regarding any elevated or abnormal tests results within 5-7 business days of your appointment.

After Hours Contacts

The on-call provider is for urgent problems only that cannot wait until the next business day. In an emergency situation, please call 911. For other problems after the office is closed, the number to call is 813-415-0100. After hours, this number is forwarded to our answering service who will contact Dr. Patel. No narcotic pain medications or other controlled substances will be called in by Dr. Patel. Routine refills will not be called in. Please respect on-call provider and make sure your call is appropriate.

In Patient Hospitals we use

If you are needed to be admitted to the hospital, please note that we will work closely and coordinate all admissions to our contracted hospitalist. This group of hospitalists are Board certified physicians and work with the major hospitals. The hospitals that we’re affiliated are Florida Hospital in Carrollwood, Florida Hospital Tampa on Fletcher Avenue, and Tampa General Hospital on Davis Island. At your discharge, we will coordinate with the hospitalist for the follow-up care at our office.

Referral to a specialist

If Dr. Patel decides that you need to go to a specialist’s office, our staff will arrange for a referral if one is required by the insurance company. We encourage that the patient discusses all the specialist’s referral at the time of the visit with Dr. Patel. He will discuss with you all the treatment options.

Internal Medicine Physician

An internist is another name for Doctor of Internal Medicine. Internist may be called general internist. They have completed training in internal medicine, and 3 years of residency training in a certified program. The residency training is solely dedicated to learning how to prevent, diagnosis, and to treat diseases that affect adults. The internist are well versed into adult preventative care medicine.
